Our Care Team

The care team at Shackleton Care Home is led by Clinical Nurse Manager TBA. TBA acts as the focal point, for our residents, family and visitors.

Our residents have the reassurance that there is always a Nurse on duty who is supported by a team of trained Health care workers. Our qualified and support staff receive regular updating and educational sessions.

The patients care is planned with the patient, the patients family and health care  professionals. Our Care plans incorporate and adhere to NICE and The Department of Health’s guidelines including High Impact Interventions to create an individual care pathway.

Each formulated care plan is regularly audited and updated as the clinical condition dictates.

Ours staff qualifications include as well as Registered Nurses. BSc(Hons) Masters
Post Graduate Educational Qualifications. Patient Assessment Qualifications. Advance Life Support. Care of the Dying Patient. Recognition of the Deteriorating Patients.ALERT Course.

We have in place a system of NEWS (patient at risk scoring) as recommended by the Royal College of Physicians 2012. To identify any problems and expediate a rapid response thus we aim to prevent admission to hospital and will work in conjunction with the patients GP and the patients wishes to achieve this.
